These are practical Rapport Marketing forms you can use to help manage your business development, improve your client communications and relationships, and your presentation skills. Use these forms as often as needed.

Instructions:

The forms are all interactive pdf files and one Word template file.

  1. Adobe Acrobat 9.0 works best. Download it for free. www.adobe.com
  2. Click on the form, do a SAVE AS and store it on your computer
  3. Open up the form and type directly into it
  4. Save the form and print it out

  • Practice Management & Business Development Questionnaire: This is an excellent questionnaire to complete in order to determine where you are in your practice and business development activities. It takes about 15-minutes to complete, and it will get you to think about what you need to do next. form
     
  • Evaluating Your Relationship Questionnaire: This is a simple questionnaire to check the status of a client relationship. Just follow the directions, it will take about 5 to 10 minutes to complete. You will also have an opportunity to decide on what you should do to improve the relationship. form
     
  • Healthy Client Relationships Evaluation: This is a quick check to evaluate how you treat all clients. There are 15 statements. Rate yourself by putting an x in your selection and add up your score at the bottom. form
     
  • Stimulating Client Relationships Questionnaire. Before you meet with a client or prospect take 5 to 10-minutes and complete this simple form that will help you focus on your discussion. It covers six areas; 1) current services provided, 2) possible needs, 3) ideas to help them, 4) questions to ask, 5) networking/referral connections, 6) action steps. form

     
  • Client Service Opportunity Report. When you are with a client and you have finished discussing current matters with them, you always need to look ahead and see how else you can help them. This form  is a prompt sheet that will help you ask the right questions to see how you can help them going forward. It covers four major areas; 1) issues to discuss, 2) action plan for what you are going to do to move forward, 3) results--how has the solution helped the client, and 4) next steps to continue and expand the relationship. form 
      
  • Target Account Strategies.This form helps you outline a strategy to pursue additional busienss from an existing client. It outlines 1) objectives for this account, 2) goals, 3) areas of reserach, 4) notes on current needs of industry and target, 5)m action plan, and 6)m implementation schedule for accomplishing everything. form  
     
  • Action Plan Form: Use this handy action plan/to-do form to organize your tasks for any project. It includes start and due dates, a section for who is responsible, and space for action plan notes. form
     
  • Daily Time Management Form: This is a handy and very flexible form (to-do list) to use on  a daily basis to make you more productive. It's simple and felxible. Try it. form
      
  • Presentation Skills for Professionals Pre-Workshop Questionnaire. Use this questionnaire to see where you are now on your presentation abilities.  form
      
  • Presentation Skills Self-Evaluation Form. Use this form to review one of your video recorded presentations and evaluate how you did. form
      
  • Presentation Skills Creating Your Presentation Word Template: Use this Word template to create and organize your presentation content. Click on the form, save it to your computer, then open up the template, and type your ideas into the empty boxes under the captions. form
